How to create a worksheet template on the computer?

I’m trying to create a worksheet on my computer that I can simply type in the information in blanks or fields that when filled in, don’t affect the rest of the info on the line.
Example:
“Name __________ Date___________ Invoice _____________”

I want to be able to Tab from field to field, filling in the Name portion without the Date and Invoice sections being pushed to the right and eventually down to the next line. I’ve tinkered around with Word Perfect and Photo shop and can’t come up with a workable solution.

Any ideas?

Use Open Office’s spreadsheet program, or Google Docs.
